- Any prizes that are sent by mail will be sent ONLY by registered mail - ONLY after we receive the information sheet back from you which is sent to the winner after the draw (If this form is not sent back and received by our office within 1-10 business days - your prize will be deemed to be forfeited and will be re-drawn at a later date)

(Ensure all personal information is correct or you may not be permitted to pick up your package - in the event it is returned to us unclaimed/refused/moved - we will assume this to be a forfeiture of the said prize)

**A winner will be drawn by random selection on September 30, 2013 @ 5:30pm and will be announced accordingly (First Name & City) on TaxSaleProperty.org and Tri-Target.com. - The winner will be emailed by our Company - You must respond in 5-10 days of the date we send the form in order to claim your prize!**

- You must have a 'Valid Email' (this is specific to your profile on www.Tri-Target.com and to be used only for the purpose of contacting the winner (If your email address is incomplete your submission will not be considered)
